The DOT physical exam includes review of medical history, a general exam, urinalysis for blood, protein and sugar, and testing of vision and hearing.
If all of the DOT requirements are met, then the driver receives a medical certificate, which can be valid up to a maximum of 2 years.
What to bring to your DOT Physical Exam and what to expect:
- Bring a valid drivers license or ID. You must know your driver license number (can be any class) if using ID for identification.
- If applicable, bring a list of all medications that you take so you can write them on the DOT form. Take all meds as prescribed. Wear your glasses and
hearing aids.
- To avoid a falsely elevated blood pressure reading, do not smoke or consume caffeine products for at least 2 hours prior to exam. The examination includes physical exam, urinalysis for blood, protein, and sugar, and if qualified, an 8.5"x11" DOT medical certificate together with a miniature water-proof copy.
Background/Credentials:
Growing up in Dallas as a child, Isaac Zinovy Pugach (pronounced POOH-gach) graduated from Richardson High School. He studied as an undergrad at Rice University, receiving a Bachelor of Science degree and graduating with Honors. Subsequently, he attended medical school at The University of Texas Medical Branch at Galveston.
After graduating in the top quarter of his medical school class, Dr. Pugach completed post-graduate Emergency Medicine internship and Internal Medicine residency at large teaching hospitals in New York City. After taking American Board of Internal Medicine certification exam and scoring in top 10% of the candidates, Doctor Pugach became board certified in Internal Medicine. He has been licensed and has practiced medicine since 2003. In 2006 he moved back to Texas, and founded Complete Med Care, a Dallas family medicine practice. In 2013 Dr. Pugach was designated as a Certified Medical Examiner, authorized to perform DOT Physicals for Commercial Drivers as a Requirement for a CDL License. English, Spanish and Russian are spoken in Dr. Pugach's office.
